How our Reablement Service adopted digital technology and processes

The Reablement Service delivers homecare support to residents discharged from hospital.

It's a complex team made up of frontline reablement practitioners and officers who manage back-office functions, who until recently were reliant on paper based processes for timesheets and care plans.

This approach created delays in processing claims and changes, with the service not being responsive and effective as it could be.

Additionally, the potential risk from data security losses and missed rota changes was greater than it otherwise may have been.

To overcome the issues identified the service commissioned a digital solution that has significantly improved practice and effectiveness.

Cloud based software enables the office team to roster and make immediate changes which has much improved the effectiveness and pace the team work at. The software also provides greater assurance for regulatory frameworks to demonstrate outcomes via enhanced reporting functionality.

Frontline staff now use an app that ensures the most up to date information is available to them, whereas previously there could be delays or a verbal handover of information. They are able to review care plans and risks as well as allowing them to log in and out of calls.

It supports lone working processes as well as being able to communicate and act on a change in the needs of the service user more promptly.

To enable this to happen the team's digital skills had to improve. This was a concern to the team when the project was first proposed however it has been overcome and the team are now appreciative of the positives the changes have brought to how they work.

We are now keen to continue to look for other opportunities to increase our digitisation.
