Local Offer: moving from children's to adult's social care

Until the age of 18, the care of children with long-term health conditions is the responsibility of child health and social care services. From 18, they are usually the responsibility of adult services. Between the ages of 16 and 18, the child will start a "transition" of the services affecting:

  • health and social care 
  • mental health
  • education
  • financial benefits for the young person and their family
  • work
  • housing

Young people with SEN and disabilities are likely to need extra help to achieve the best possible life outcomes. Schools will start planning transition to adulthood with the young person and their family from the age of 14. This will need to involve other agencies including careers guidance organisations who offer careers advice and advice on further education and training.

Some young people with severe disabilities may be entitled to support from Adult Social Care.
 

West Berkshire Council's Housing Team will be able to advise on whether young people are entitled to help with housing.
